The World Heavyweight Championship is currently the longest running WWE title belt without a major design change, its been effectively the same since its debut in 2002 till now. The US Championship is second.

I felt that the WHC got smaller over the years, I'm not sure if that's true or not but it just looks smaller to me

Just having a quick google image of different champions with the belt, I don't think it has got smaller. Triple H's title looks the same size as Sheamus'.
